Participation Forms
Lee Middle School offers competitive extra curricular opportunities in athletics for both 7th and 8th grade students. The emphasis for middle school sports is focused upon the development, practice and mastery of individual fundamental sports skills; the development of good team game play; and the promotion of good sportsmanship. All students in attendance at Lee have the opportunity to try out for these competitive teams provided they meet the following minimum requirements:
- students must maintain the minimum grade point average (2.00) and receive no failing grades
- all students must complete the following forms and turn them in by the deadline
- Athletic Participation Form, the WJUSD Athletic Participation Handbook
- Covid - 19 Safety Form
- Release of Liability Form
- All students must have insurance
- Students and families may purchase an athletic insurance policy from the Student Store
Our programs are competitive in nature, so some teams will have tryouts and cuts. The teams that do not have tryouts are cross country, track and field and wrestling. The following forms must be filled out and turned in to the Athletic Director in order for students to be eligible to tryout for athletic teams for the current school year.

If your child makes a competitive team you will need to fill out the following forms that fit your situation for transportation needs:
Volunteer Personal Auto Form - to be filled out if you plan to drive students to athletic events.
Spanish Volunteer Personal Auto Form- Spanish version - to be filled out if you plan to drive students to athletic events.
Student Alternate Transportation Form - to be filled out if you do not plan to drive your child to all athletic events.
Spanish Student Alternate Transportation Form - Spanish version - to be filled out if you do not plan to drive your child to all athletic events.
All parents planning to drive students to athletic events MUST go through the District's Volunteer Clearance process.
